Bringing a friend to Youth Ministry Night for the first time?  Download and print the Visitor Emergency Contact form and bring it with you!  We want to be sure we have a way to contact their parents if they get hurt during re: Creation or in case of an emergency.

Don't have a current permission form on file?  Did you change addressed or doctors?  Update your emergency information by downloading and returning the 2010 Youth Participant Permission form.
